Complete and Submit Committee Forms
If you are assigned to respond to a committee form at the current case review step, you can find it under ‘Required Items’ on the case’s ‘Case Details’ tab. Depending on how the form was configured, committee managers, committee members, or both may be asked to respond. You can save unfinished responses and return later. A submitted response may also be editable by its original respondent.
A committee member given access to a case with required forms receives an email with a note about the items they need to complete as part of the review.
No one can submit a response on another reviewer’s behalf.
Complete Committee Forms
- Navigate to the Cases page and open the case you're reviewing. Select the Case Details tab, and then scroll down to the ‘Required Items’ section and click Fill Out Form.

Committee managers may also see Manage respondents for forms at their assigned case review step. This control lets them review and adjust response requirements; it does not let them submit on another reviewer’s behalf.

- Answer the questions. Select Save Responses to keep a draft without submitting it, or Submit Form when your response is ready.
- After submission, select the Edit Submission button to revise your own response.
FAQs
Why does “Missing Committee Materials” still appear after I submit my committee form?
This notice reflects outstanding required items for the case review step, not just your response. Another form response or required document may still be missing. If minimum response counts are set, the form stops blocking advancement once each selected respondent group meets its required count, even if other eligible respondents have not submitted.
To check your own form, open Case Details > Required Items > Forms. ‘Edit Submission’ indicates that you submitted a response; if you still see ‘Fill Out Form’ open it to check whether you saved a draft instead.
