About Forms & Reports

The Forms & Reports page gives faculty access to Custom Input Forms, Initiated Activity Input Forms, and institution-generated reports to support activity management. Administrators can also use this section to generate reports for informational purposes, including Prior Activity Input Forms (previously submitted forms) and My Activities Reporting, which displays all activities entered in FAR with filters like teaching metrics, grant status, date ranges, and classifications.

 

Forms & Reports

  1. Complete the Custom Input Form or Initiated Activity Input Form.
  2. You can view Prior Activity Input Forms and generate My Activities Reporting under the Reports section.
  3. Navigate to the “My Activities” Reporting page, select the desired filters, and then click Refresh Report.
  4. Scroll down to view the breakdown of the report.
  5. Click the hyperlinked Total or summary value to dive deeper into it.
