Create Custom Input Sections
Data entered into the Profile Form and Activity Input Form provides the foundation for generating multiiple outputs. This article explains how administrators can customize input sections for Profile and Activity Input Forms within FAR.
Create Custom Input Section
- Navigate to Setup > Activity Input Form > Edit and the Configure Form page will display.
- Under the Content section click Add Section to create a custom input section. You can also show, hide, or reorder sections here.
If a section dropdown has Reorder next to it, it indicates a Default Section, meaning its fields cannot be configured (though activity classifications and section settings can be modified). Only sections labeled Custom Section Reorder have configurable fields.
- You can also click the Pencil (Edit) icon to edit an existing section.
- The Add Section: Level page will display. Fill out the form by following the steps in the table below:
Where Do These Changes Appear?
When a custom section is created, it appears in all Activity Input Forms at the same unit level but is initially set to 'Do Not Show' in existing forms. Changes to a custom section will be reflected wherever it is displayed, regardless of the unit.

Section Fields |
The default subsection name is Input Form Click the blue arrows to edit Subsection and Field information. Add Label to Vita Output ExampleIf the field label is Number of Hours and the value is 10, the vita output will show Number of Hours: 10 if this box is checked. Set Default Value & Limit Number of Characters CheckboxesIt is recommended to check both boxes next to the Set Default Value and Limit Number of Characters fields. If these boxes are selected, they cannot be left blank or the form will not save. |
Activity Classifications |
Identifies how and where to use activity classifications, which can serve as tags/filters in reports. Use drag-and-drop to select the desired classification. |
